I have included a plan for my current patient’s with multiple children. The plan includes a yearly well visit for each enrolled child. All well visit related in-office labs are included. The plan requires a 1 year commitment. All vaccines will be billed directly to patient’s insurance.

Additional sick visits, well check ups and telemedicine visits are included. Additional in-office labs and in-office treaments are also included

For 2 children the rate is $250/month, each additional child is $50/month

This fee will be applied toward the first month of the plan.
